The Inclusa Foundation is devoted to fostering diverse and inclusive communities for older adults and adults with disabilities. Through the lens of Commonunity®, our unique model of support, we leverage valued programs, impactful partnerships, and innovative practices to transform communities across Wisconsin.

A rich history in Wisconsin

The Inclusa Foundation was formed from the proceeds of the acquisition of Inclusa, Inc.’s assets by Humana. The acquisition provides funds that will support grant making opportunities and sustain the Foundation for the long-term.

  • Support for development of collaborative approaches and outcome driven funding models to address key issues of the day: direct care workforce crisis.

  • Support for innovative service delivery through transformative provider practices for people with disabilities and older adults.

  • Support for development of value-based purchasing approaches that focus on outcomes for people with disabilities and older adults.

  • Support for programs and offerings that align with Commonunity® with attention to individual choice, living in one’s own home or apartment, making valued contributions, having full access to the community, and building a sense of belonging.

  • Support for public policy development aimed at providing enhanced outcomes for people with disabilities and older adults.

  • Support for ongoing development of information and education that helps self-advocates and advocacy organizations be more impactful in their work.
